A Clinicl-Pathological Study and Chemotherapy for Maligant Ovarian Tumors



Abstract
A clinico-pathological study was made on a series of 7 maligant ovarian tumors which were operated and confirmed by postoperative histopathologic study at Obstetrics and Gynecology Department of Kyung Hee University Hospital during a period of 6
years
and 8 months from November 1987 and June 1994.
@ES The results were obtained as follows:
@EN 1. Age distribution of malignant ovarian tumors showed the highest frequency in 5th decade and the average age was 43.2 years old.
2. The frequency of nulliparity was 27.2% and the primary infertility rate was 4.6%.
3. Previous menstrual cycles of menstruating group has been abnormal in 25.0%, postmenopause in 36.4% land prepuberty in 1.3%.
4. As the subjective symptoms of all malignant ovarian tumors, lower abdominal pain in 35.0%, palpable abdominal mass in 24.7%, abdominal distension in 20.8%, and 9.1% of patients was not complained. The interval from onset of symptoms of
diagnosis
was
75.3% in 6 months or less.
5. Positive cytology of ascites or peritoneal washing fluid was seen in 23.2%, and sensitivity of ascites in 33.3% and specificity in 95.0%.
6. metastasis of omentum was 31.0%, 35.3% for peritoneum, 13.0% for pelvic lymph node, 12.5% for paraaortic lymph node, 8.2% for parametrium and 6.6% for salpinx.
7. Bilaterality was noted in 22.1% by gross or microscopic examination.
8. Histologic incidence was as follows: malignant epithelial tumors-in 70.1%, germ cell tumors in 15.6%, sex cord stromal tumors in 13.0% and metastatic tumors in 1.3%, and of malignant epithelial tumors, mucinous carcinoma in 59.3%, serous
carcinoma in
31.5%, endometrioid carcinoma in 7.4% and undifferentiated carcinoma in 1.8%.
9. According to FIGO staging of primary ovarian tumor, stage I (69.8%), stage II (2.6%) stage III(18.4%) and stage IV(9.2%).
10. Tumor markers of CA-125, CA 19-9, AFP and ¥â-hCG were increased in 66.7%, 47.4%, 17.5%, 27.3% and 12.5% of the selected patients, respectivity.
11. Of 77 cases, total abdominal hysterectomy with bialteral salpingo-oophorectomy was performed in 58.4%, and omentectomy in 54.5%.
12. Combination chemotherapy was performed on 43 cases(55.8%), and of 43 cases, 59.3% for epithelial tumors, 66.7% for germ cell tumors, 20.0% for sex cord stromal tumors and 100% for metastatic tumor.
13. Response rates by combination chemotherapy were 69.2% by CAP regimen, 62.5% by CP regimen, 75.0% by Cc regiment and 80.0% by BEP regimen.
